Activity Work Plans Adelaide PHN - An Australian Government Initiative

Activity Work Plans

Based on allocated funds from the Australian Government, the Activity Work Plans inform how Primary Health Networks (PHNs) address the health and service needs identified through the needs assessment process.

Together with the PHN Needs Assessment and the PHN Performance Framework, PHNs will outline activities and describe measurable performance indicators to provide the Australian Government and the Australian public with visibility as to the activities of each PHN. 


Thus far, PHNs have completed three Needs Assessments – the Baseline Needs Assessment (BNA), completed in March 2016 and an update to the BNA was completed in November 2016 ( Adelaide PHN Baseline Needs Assessment Update Report 2016/17) and the most recent (called the Needs Assessment Update) in November 2017. 

The following Adelaide PHN AWPs were submitted in 2018 and approved accordingly. 

 

Adelaide PHN Operational & After Hours Updated Activity Work Plan 2018/2019

The Operational & After Hours Updated Activity Work Plan covers the period from 1 July 2016 to 30 June 2019. This document, the updated Activity Work Plan template, captures those (existing, modified and new) activities addressing needs identified in the updated BNA. It will identify local priorities which in turn will inform and guide the activities to correspond with the PHN Objectives, Key Priorities and/or the National Headline Performance Indicators. Additionally the After Hours Activity Work Plan address gaps in after-hours service provision and improve service integration within the PHN region.

Adelaide PHN Mental Health Activity Updated Work Plan 2018/2019

The Mental Health Updated Activity Work Plan will help guide activities and outline the planned mental health services, reflecting priorities as identified in the updated Needs Assessment, to be commissioned for the period from 1 July 2016 to 30 June 2019. This document, the updated Activity Work Plan template, captures those (existing, modified and new) activities addressing needs identified in the updated BNA.

Adelaide PHN Drug and Alcohol Treatment Updated Activity Work Plan 2018/2019

The Drug and Alcohol Treatment Updated Activity Work Plan will help guide activities and outline the planned Alcohol and Other Drugs (AOD) treatment services, reflecting priorities as identified in the updated Needs Assessment, to be commissioned for the period from 01 October 2016 to 30 June 2019. This document, the updated Activity Work Plan template, captures those (existing, modified and new) activities addressing needs identified in the updated BNA.

Adelaide PHN Integrated Team Care Funding Updated Activity Work Plan 2018/2019 

The Integrated Team Care Updated Activity Work Plan outlines the Adelaide PHNs direction for commissioning of the Closing the Gap program from 1 July 1 2018 to 30 June 2021. Adelaide PHN will work with the provider to implement flexible approaches to best meet local community needs, with the aim to improve Aboriginal and Torres Strait Islander peoples access to culturally appropriate health care.

Adelaide PHN Greater Choice for At Home Palliative Care Activity Work Plan 2018/2019

The Greater Choice for At Home Palliative Care (GCfAHPC) Activity Work Plan covers the palliative care component of Core Funding provided to PHNs to be expended within the period from 1 January 2018 to 30 June 2020. Through a Commonwealth Department of Health Expression Of Interest (EOI) process undertaken in August – September 2017, all 31 PHNs were invited to submit their interest in implementing the Greater Choice for At Home Palliative Care (GCfAHPC) pilot measure. Through this process, Adelaide PHN is one of the 10 PHNs were selected to receive funding to implement the measure. The GCfAHPC provides funding to improve coordination and integration of end-of-life care across primary, secondary, tertiary and community health services to support at home palliative care through funding PHNs.

 

Adelaide PHN National Psychosocial Social Measure Activity Work Plan 2018/2019

The purpose of the National Psychosocial Support (NPS) measure is to provide psychosocial support services to assist people with severe mental illness resulting in reduced psychosocial functional capacity who are not more appropriately supported through the National Disability Insurance Scheme (NDIS).  These services, in partnership with families and carers (as appropriate), will provide a range of non-clinical community based support to these individuals to achieve their recovery goals.  The NPS measure is being implemented through purpose specific funding to Primary Health Networks (PHNs) to commission these new services for 2018-2019.  

 


 

The key objectives of Primary Health Networks (PHN) are:

  • Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
  • Improving coordination of care to ensure patients receive the right care in the right place at the right time.
